The Ogikubo Tadashi Clinic is a special clinic for home visits located in Ogikubo, Suginami-ku, Tokyo. This clinic is unlike your usual hospital that sees outpatients. It provides 24-hour medical care where doctors visit the homes and facilities of mainly chronically-ill patients who are unable to visit the hospital due to various reasons.

The director of the clinic, Dr. Tadashi Abe, previously worked for a hospital as a neurosurgeon.

”In current acute phase* treatment, since patients are often discharged or transferred to another hospital two to four weeks following an operation, the doctors lose track of the patients afterwards. As a doctor, I was always concerned about my patients after they left the hospital, and I had a strong urge to become more involved in the community and that’s why I opened my clinic in 2005.” (Dr. Abe)

* Acute phase: acute symptoms/signs develop, patients are in critical condition and the course of the illness is short. Complications from operations develop quickly and whole body management is necessary during this phase.

Keenly aware of the limits of paper-based medical records, Dr. Abe introduced electronic medical records and the CF-H1.

When the clinic first opened, paper-based medical records were used and Dr. Abe would regularly place the paper-based medical records for 10 or so patients in his bag in order to visit these patients. Later, after returning to the clinic, he would write up his reports by hand daily.

However, it was not uncommon for patients to unexpectedly suddenly contact the clinic. Dr. Abe stated he was keenly aware of the limits of paper-based medical records in these cases.

”Paper-based medical records are not very light. There is one file per patient, and if a patient has been receiving treatment for a long period of time, that patient’s file can become very thick. Therefore, bags like those used for traveling are needed to carry around all the medical records of 10 patients.

What was even more serious was that I didn’t have the medical records with me when a patient would contact us suddenly. I had to work from memory or I had to call the clinic to confirm information on the medical records. It wasn’t very good and high quality medical treatment wasn’t realized. What’s more, when I thought about when the number of doctors in my clinic would increase, there would be a large difference among the individual doctors and paper-based medical records would not suitable for standardization and information sharing would be poor. For these reasons, I wanted to introduce an electronic medical record system which enabled the medical record information of all patients to be viewed anywhere.” (Dr. Abe)

Therefore, the clinic introduced the “m-WAVE” home visit electronic medical record system from Digital Office Co., Ltd. in 2010 and the use of CF-H1 as terminals for home visits began.

Following the introduction of electronic medical records, the format of the visits was greatly transformed. Prior to the visit, the medical records of all patients within the server are transferred to the CF-H1. Later, Dr. Abe is able to leave with all necessary medical equipment, the CF-H1, and a Bluetooth portable printer.

“For facilities (such as nursing homes) in particular, since I see a large number of patients all at once, I often print and hand out prescriptions and referrals right in front of the patient. The CF-H1 is very convenient since it can be connected to the printer via Bluetooth.” (Dr. Abe)

Dr. Abe also said that the small size of the CF-H1 and operability of the touch panel cannot be compared to other laptop computers even though the same functions are available in them.

“You have to place a laptop computer somewhere flat, and often there isn’t much space in a patient’s home or a care facility. With the CF-H1, there’s a unique hand-strap on the bottom, so I can explain the test results while holding the CF-H1 in one hand. The CF-H1 also recognizes handwriting, so it’s very convenient.” (Dr. Abe)

Dr. Abe remarked that he can respond appropriately without hassle and without depending on the communication environment even when he receives an emergency call from a patient, because the medical records for all patients are in the CF-H1.

When Dr. Abe returns to the clinic after his home visits for the day, he sets the CF-H1 in its cradle and synchronizes the data with the server. Dr. Abe says that this method is effective from the BCP* aspect as well. Even if equipment at the clinic becomes unusable or communication is not available because of a large disaster, the electronic medical records can still be used with the CF-H1.

“During a disaster, being able to refer to a patient’s medication history is useful even though I may not be able to provide sufficient treatment. For example, if I can provide insulin to a patient with diabetes, that would be a great help.” (Dr. Abe)

In order to take electronic medical records out with the CF-H1, double and triple security measures are taken. Firstly, fingerprint authentication is adopted for logging in and accessing the OS and electronic medical records. The fingerprints of Dr. Abe and the other part-time doctors are registered, and not only is the input of IDs/passwords simplified, but logging in by a third person is not possible even if the CF-H1 is lost or stolen.

In addition to hard disk encryption, the CF-H1’s hard disk protection function disables content viewing of the hard disk when it is removed from the CF-H1 and connected to another computer.

Furthermore, features such as its robust design and long battery life also make the CF-H1 suitable for home visits.

“Since doctors on home visits go to various places, there is always the risk of dropping or bumping the terminal into something. The terminal can also get wet due to rain as well, so, regular laptop computers are not adequate.”

”For example, if I take a picture of a bedsore and register the picture in the medical records, higher quality consultation will be possible. In the CF-H1, photos can be transferred to the medical records right after the picture is taken without any problems or mistakes.” (Dr. Abe)

Dr. Abe said it would be ideal if a complete network among hospitals, clinics, business offices for senior care and pharmacies is built where data sharing becomes possible because the CF-H1 is used.
